Within Swansea Bay University Health Board there are eight LCCs.
Our clusters are made up of groups of GP practices, dentists, community pharmacies, local opticians, community nurses, and allied health professionals.
They work together to pool resources and share best practice in a bid to help you remain fit and healthy, and to improve the way you are cared for if you do become unwell.
Wherever possible, the cluster will try to accomplish this in the heart of your community, sparing you the trouble of having to travel to hospitals or central clinics.
Professionals from GP practices, dental practices, community pharmacies, local opticians, community nurses, and allied health professionals in each cluster work together to identify the population needs in its area.
They then devise a three-year plan to meet those needs concentrating on areas, such as:
A Pan Cluster Planning Group oversees and coordinates the work of the clusters.
The group is made up of partners from Swansea Bay University Health Board, Swansea Council for Voluntary Services, Llais (formerly known as Community Health Council), local authorities and the Regional Partnership Board.
Each of the clusters has its own webpage (accessible on the left-hand side of this page) where you will find more information about the services and projects that they are currently working on.

There are eight LCCs within the Swansea Bay region. They are:
Afan – which serves a population of 50,845.
Bay Health – which serves a population of 75,221.
City Health – which serves a population of 54,806.
Cwmtawe – which serves a population of 42,680.
Llwchwr – which serves a population of 49,075.
Neath – which serves a population of 56,500.
Penderi – which serves a population of 37,443.
Upper Valleys – which serves a population of 32,138.
