Pan Cluster Plan summary 2024/25

Introduction

The introduction of the Accelerated Cluster Development (ACD) programme, which began in April 2022, is designed to increase the speed of implementation of the Primary Care Model for Wales (PCMW) and improve collaboration across all agencies to improve the quality of health and social care for the public.

One of the key elements of the programme was to establish a regional Pan Cluster Planning Group (PCPG), underpinned by our eight Clusters – known as Local Cluster Collaboratives (LCCs). The PCPG sees staff work together to deliver the aims of the Social Services and Wellbeing Act 2014, The Wellbeing of Future Generations Act 2015 and A Healthier Wales.

The PCPG identifies and agrees planning priorities based on population health needs, including feedback and input from the eight LCC plans, along with the funding and service levels to deliver the objectives.

This has developed into a formal but flexible Pan Cluster Plan, which is agreed by both the Regional Partnership Board (RPB) and Swansea Bay UHB (SBUHB) executive management board.

The Plan covers objectives from the eight LCC plans (summaries of which will be published individually), SBUHB Recovery & Sustainability Plan, RPB Area Plan and the Public Service Board Plan, amongst others.

It seeks to broaden and strengthen clinical and social engagement and to increase influence from the community. It therefore contains a number of projects that align with the delivery of common regional priorities across health and care services.

The purpose of this paper is to summarise our Pan Cluster Plan for 2024/25, which has been approved by the PCPG, which builds on the successes and achievements of previous plans and sets out our strategic direction for the next year.

2024/25 Pan Cluster Plan

Our 2024/25 Cluster Plan was formally signed off at the end of March 2024 and has six key priority areas as follows:

1. Learning disabilities

- What is the Goal? Improving outcomes for those with learning disabilities, ensuring they receive the right care and support.

- How? Improve access to and information on local services including annual health checks. Promote accessible positive lifestyle behaviours including health eating, exercise and improving accessible opportunities in the community.

2. Carers

- What is the Goal? Improved outcomes for carers.

- How? Improve early identification, support and signposting for carers across Primary and Community services areas. Collaborate with carers, service providers and Carers Partnership to understand needs and create a plan to help upskill community service providers, incorporating carers support initiatives into local services, informed by best practice.

3. Improve care for older people

- What is the Goal? Maximise cluster-based models of care to support the health, wellbeing and independence of older people in their homes wherever possible.

- How? Support Clusters to enhance, develop and implement community-based services to support older peoples’ physical and mental health. Enhance links between Primary Care contractors, Cluster teams and wider community services such as Virtual Wards, District Nursing, Allied Health Professionals, Acute Clinical and third sector organisations.

4. Emotional and Mental Wellbeing (EMWB)

- What is the Goal? Deliver individually focussed services enhancing the strengths of communities to improve emotional and mental wellbeing of the public.

- How? Joint work on the implementation and development of Emotional and Mental Wellbeing Strategy with LCCs and RPB collaborating to design services. Implementing a new model of Community Psychology provision across four clusters.

5. Strategic Workforce Plan for Primary Care

- What is the Goal? Ensure that we have a sustainable workforce for primary and community services in place across the eight LCCs over a 10-year period up to 2035.

- How? Once the national Strategic Framework is released in May we will collaborate with the SBUHB Primary Care Academy to develop local plans to reflect the key themes of the framework.

6. Review services for vulnerable groups

- What is the Goal? Ensure we provide a proportionate approach to equity of delivery of healthcare provision for vulnerable groups e.g. specific patient groups with poorer outcomes.

- How? There are people who experience exclusion form routine primary care services in every area of Wales so we need to review arrangements under an Inclusion Health Service for those that are homeless or at risk of homelessness / asylum seekers / refugees in line with national specifications.

Next steps

We will develop detailed implementation plans with key stakeholders for each of the six priorities. Both the Strategic Workforce Plan and Health Inclusion Plan are pending further information from the national team and will need further consideration once full details are available. We will also be publishing summaries of the eight individual Cluster Plans which partially inform this document. 

Reviewing, reporting and monitoring

The Pan Cluster Plan will be implemented in a structured way and will be overseen by the PCPG. This will include monitoring progress against the eight local LCC plans, alongside making sure it aligns with SBUHB and RPB plans.
