Fair and equal access to Audiology service.

Re-designed repeat medication ordering.

Supporting uptake of public health screening programmes.

Supporting increased uptake of smoking cessation.

 

Providing a timely and responsive home visit by a Community Clinician rather than a GP.

Continuing to develop of cluster led Primary Care MH model.

Increasing Social prescribing for patients with low level mental health and social issues.

Improving access to Child and Adolescent Mental Health services (CAMHS).

Support the roll out of Local Primary Mental Health Support Service (LPMHSS) Link Worker.

 

Improve outcomes for the most vulnerable families.

 

Delivering an enhanced Flu Vaccination Programme.

Commence delivery of Primary and Community Services joint RPB Carers Plan for improving Carers' early identification; signposting; and support across agreed service areas of each LCC.

Enhancing patient well-being by fostering the development of healthier lifestyles and behavioural practices with Third Sector.
