The Community Resource Team (CRT) is a joint service provided by this health board and Neath Port Talbot council social services.

The team helps adults who are living in the Neath Port Talbot area with the support they need to retain their independence in their own homes.

This support can include acute clinical care, home alterations and equipment, physiotherapy and speech and language therapy, help with diet, help with washing and dressing and help with medicines.

How to get help

The team can support people to come out of hospital and back to their own homes. They also help to prevent hospital and care home admissions by assessing the level of need and providing the right assistance.

GPs, hospitals, district nurses and social workers are among those who can refer people to the CRT.

Patients, their families or their friends can also contact the team directly through the Neath Port Talbot Gateway. This is open Monday to Thursday between 8.30am and 5pm and Friday from 8.30am to 4.30pm.
