  • Where do we work and how?

The Community Memory Support Team is for those living in Neath Port Talbot who are experiencing memory issues or changes to cognitive function which can affect understanding, reasoning, learning and problem solving.

Many people experience memory loss as they get older. This is not necessarily due to dementia or Alzheimer's disease. It could be due to hearing loss, depression or isolation.

The multi-skilled team, which consists of dementia practitioners and memory support workers, visit people in their own homes to undertake an assessment with a view to providing early intervention, support and information for the patient, their carers and family members.

The assessment involves one or more home visits by a member of the team who will discuss any difficulties and carry out a variety of cognitive assessments.

If a memory or cognitive impairment is identified the team will liaise with the patient's GP to arrange further screening. 

If they identify other issues during the home visits, they will ask for consent to refer to other services which may include the older adult mental health service, social work teams, Age Cymru, NPT Carers' Service, the fire service, befrienders or reablement service.

 

This is a free service for those aged 65 and over living in Neath Port Talbot who have not had a formal diagnosis of dementia.

 

Anyone can make a referral to the Community Memory Support Team such as health professionals, family members, carers and individuals who are concerned.
