Complex needs - Service for patients with at least two of the following mental health, substance misuse, domestic violence.

Commissioning of a varied range of emotional and wellbeing services in the Cluster.

Psychological Therapies.

Community Psychologist investment.

Fertility - Minimising waiting lists in secondary care and offering tailored lifestyle advice.

Suicide Prevention - new practice coding template to inform service development.

 

Develop a peer support group across the LCC.

Co-producing outcomes to support patients to manage healthy eating and weight management.

Further work on co-produced needs assessment from Care homes and their residents.

Commence delivery of Primary and Community Services joint RPB Carers Plan for improving Carers' early identification; signposting; and support across agreed service areas of each LCC.

To work collaboratively with Swans Foundation to set up Community Programme focussing on healthy behaviours.
