Outpatient follow ups

Outpatient care is being revolutionised, with the aim of reducing unnecessary routine appointments and offering more opportunities for patients to decide for themselves if and when they need to be seen.

New ways of working give many patients and their carers the opportunity to initiate their own appointments as and when they need them. This might be when they have a flare up of their symptoms, or change in their circumstances, rather than having their appointments booked at routine intervals as is traditionally offered. This helps avoid unnecessary routine appointments and makes it easier for patients to book appointments when they really need them.

As well as empowering patients to be more involved in the management of their condition, and take responsibility for arranging the appointments they need, another benefit is that shared decision-making between patients, carers and clinicians is better supported. Manage your healthcare from the comfort of your own home

Do you want access to your health information at your fingertips? Manage your healthcare from the comfort of your own home? Get a range of test results sent directly to your phone, laptop or PC? Feel more involved and in control?

The Swansea Bay Patient Portal is a secure online service which gives you access to your health information, and it’s convenient – you can access it anytime and anywhere. It can also provide you with a library of information resources like websites, leaflets, audio files and videos provided by your clinicians. You can also upload your own details, for example diagnoses, allergies, medications, symptoms, and measurements such as pulse and blood pressure readings.

The Swansea Bay Patient Portal offers you a great range of opportunities to self-monitor your condition, and in many cases you can also *direct message health professionals involved in your care. You can also share some or all of your health information with family members, carers or other health care professionals who need to see it. *Please note: Direct messaging may not be available in some services – check with your clinical team first.

Free training to help you manage your long-term condition

Nobody knows yourself better than you do.  So it makes sense that the same will be true when it comes to managing your health condition, or caring situation.

Education Programmes for Patients are self-management programmes which complement the care you get from the NHS, by giving you the ability to manage your health condition.  The programmes have been shown to enable patients to take control of their health and wellbeing by learning a set of tried and tested techniques. These increase confidence and the ability and belief to cope and manage health conditions, leading to an improved quality of life.
