What is my Patient Account and my SBUHB Patient Record on the SBPP?
Information which falls into the definition of part of your SBUHB Patient Record on the SBPP includes information;
- contributed by SBUHB,
- input by yourself or another organisation which is shared with SBUHB with, or
- input by yourself following encouragement or direction received from SBUHB, for which SBUHB is a data controller.
Where you decide to share information within the SBUHB Patient Record with another organisation, the information also forms part of the recipient’s Patient Record. The two Patient Records are treated as two separate records.
Where information which originates from another data controller (such as another Health Board) is shared by yourself with SBUHB, this becomes part of the SBUHB Patient Record. The two Patient Records are treated as two separate records.
A Patient Account includes all information on the PKB Solution assigned to that patient. The SBUHB Patient Record forms part of your Patient Account.
