Welcome to the page for the primary care clusters
There are eight clusters consisting of independent contractor services: GMS, GDS, pharmacy and optometry.
There are five clusters within Swansea: Llwchwr, Bay Health, City Health, Cwm Tawe and Penderi and three clusters within Neath Port Talbot: Upper Valleys, Neath and Afan.
There are two Community Resource Teams (CRT), one supporting Swansea and the other supporting NPT.
Services provided across the clusters include District Nursing (DN), Acute Clinical Team (ACT), GP out of hours (GPOOH), continence, speech and language therapy, audiology, occupational therapy, outpatient services, physiotherapy, sexual health, wound care, podiatry, MCAS, school nursing and LAC, Swansea north, west and central hubs for DN, Virtual Wards and Home First.
