This service supports hospital discharge, delivering the new Rapid Discharge to Assess Service.
It has a multi-disciplinary team including occupational therapists, physiotherapists, community discharge liaison nurses, social worker/care management officer, senior care staff and business support officer. Both common access point and Rapid Discharge to Assess teams operate the ability to undertake same day response, where appropriate.
The therapy team consists of occupational therapists (both local authority and SBUHB employees), physiotherapists, speech and language therapy (SLT), dietetics, profession-specific assistants and generic support workers.
The service runs a targeted weekend service, to respond to both urgent community referrals and hospital discharges.
The service is located across three hubs: West and Central, the Civic Centre in Swansea and North Hub in Gorseinon Hospital.
ALL referrals must be submitted via Signal.
