The A-Z list below includes many services that support discharge from hospital. Also listed are services which can help with related issues such as abuse and homelessness.

 

Further guidance for staff

Taking early steps to engage and consider appropriate pathways can ensure the right patients go to the right place at the right time, potentially avoiding huge delays in their discharge from acute care.

This guidance has also been designed to support hospital staff during board rounds with embedded SAFER principles and with an emphasis on the National Discharge to Recover and Assess pathways 0-3.
