Improve diagnosis and management of Hypertension.

Improve Antimicrobial prescribing.

Engage in prescribing management schemes & further improve pain management.

Improve uptake of diabetes education program.

Increase the use of Consultant Connect.

Improve End of Life (EOL) for patients and patient’s family.

 

Promote the use of the Common Ailment Scheme (CAS).

Raise awareness of ‘Choose Pharmacy’.

 

Develop and provide consistent psychological services.

Continue to develop the Social Prescriber and Mental Health Practitioner roles.

Improve access to Child and Adolescent Mental Health services (CAMHS).

Deliver annual health checks for people with learning disabilities.

Commence delivery of Primary and Community Services joint RPB Carers Plan for improving Carers' early identification; signposting; and support across agreed service areas of each LCC.

 

 

Improve uptake of childhood vaccinations.

Work with Fertility network to introduce a cluster service.

 

 

Raise awareness of Public Health screening programmes.

Deliver an enhanced Flu vaccination programme.

Support the reduction in the prevalence of smoking.

Reduce the environmental impacts of inhaler use.

Support individuals who are frail, socially isolated or at risk of falls.

Enhance patient well-being by fostering the development of healthier lifestyles and behavioural practices involving Third Sector.
