Cancer is a condition where cells in a specific part of the body grow and reproduce uncontrollably. The cancerous cells can invade and destroy surrounding healthy tissue, including organs.

Cancer Information and Support Services (CISS) is a registered charity which offers emotional support and accurate, up-to-date information for cancer patients and carers across south-west Wales.

The charity aims to provide a flexible cancer support service, offering emotional support and factual information, while avoiding possible barriers to open discussion.

Care & Repair Western Bay's mission is to ensure that all older people are supported to live in a safe, warm and secure home as independently as possible for as long as possible.

It is a person-centred and problem-led service, which provides a range of advice services for older people in the Swansea and Neath Port Talbot areas.

Discharge medicines review

After coming out of hospital, get a review of your medication with a pharmacist to discuss any new medicines and dose changes, possible side effects, interactions with other medicines and the best time to take them.

Sore Throat Test and Treat

This screens people for bacterial or viral throat infections and allows pharmacists to prescribe antibiotics or painkillers, respectively, if appropriate.

Stop smoking scheme

Community pharmacies now offer help to people trying to quit smoking. This scheme gives advice and motivation to people struggling to quite and provides a variety of nicotine replacement therapy to minimise the number of cravings they get.

Medical waste disposal

Patients can bring unwanted or used medication back to the pharmacy to be disposed of safely.

The Dementia Hwb is a one-stop information centre anchored in the heart of the community for all things dementia related.

It has a permanent base in Swansea's Quadrant Shopping Centre between 11am and 3pm, 7 days a week.

The Dementia Hwb offers information about dementia and signposting to appropriate support.

It has introduced mobile hwbs across Swansea and Neath Port Talbot, with sessions held at the Forge Fach Community Centre in Clydach, Asda Llansamlet and Clydach Library within the Cwmtawe Cluster.

Fibromyalgia, also called fibromyalgia syndrome (FMS), is a long-term condition that causes pain all over the body.

The symptoms of fibromyalgia are changeable – for example, they can sometimes suddenly improve or get worse.

Although there's currently no cure for fibromyalgia, there are treatments to help relieve some of the symptoms and make the condition easier to live with.

If you have fibromyalgia, support groups can provide an important network for talking to others living with the condition.

Maggie's is a charity that provides free expert cancer care and support in centres across the UK, with one based in Swansea's Singleton Hospital.

The charity offers everything from help with money worries, support groups and courses and workshops, among much more.

Call 111 press option 2

24/7 mental health support is now available over the phone in Swansea Bay. People living in Swansea and Neath Port Talbot who need urgent support with mental health issues can now call a team of mental health professionals for free, day or night.

Calling 111 and choosing option 2 puts callers in direct contact with a team of 20 mental health professionals based at Neath Port Talbot Hospital.

The service is available for anyone in Swansea and Neath Port Talbot with a mental health concern, including relatives who need advice.

Call 111 Option 2 is running 24 hours a day, seven days a week, offering a triage service and support or signposting as appropriate.

Callers will be supported by fully trained clinicians working as part of a multi-disciplinary team including mental health nurses, psychological wellbeing practitioners and occupational therapists.

The Primary Care Cluster Occupational Therapy Team comprises:  

  • Katy Silcox, Senior Occupational Therapist  
  • Rowan Pengelly, Occupational Therapy Technician  

 

The team operates Monday to Wednesday across the following locations:  

  • Clydach Primary Care Centre  
  • Sway Road Surgery  
  • New Cross Surgery  
  • Strawberry Place Surgery  
  • Llansamlet Surgery  
     

What do Occupational Therapists (OTs) do? 

Occupational Therapists (OTs) provide practical support to help individuals engage in activities that matter to them. They assess a person's strengths, functional abilities, and health care needs, identifying what they want and need to do. OTs consider whether the environment; home, workplace, or community supports or hinders an individual's ability to do these activities. They work with them, to find ways to make daily tasks and activities more manageable. This support may include:  
 

  • Managing tasks within the home  
  • Accessing and doing the things that matter to you in your community 
  • Finding or staying in work/education  
  • Dealing with how the symptoms of a health condition affect your everyday life. 

What does the Cluster Primary Care Occupational Therapy Service offer? 

The service provides holistic and proactive OT assessments, support, and follow-up, tailored to individual needs. Support can take place in your home, community, or GP surgery. Using a “what matters to me” approach, the team identifies issues, barriers, and strengths in collaboration with individuals and their families. Patient-centered goals are set and worked towards through short-term interventions, including: 
  

  • Supported self-management of chronic conditions  
  • Equipment provision  
  • Falls assessment  
  • Relaxation techniques  
  • Cognitive review  
  • Functional assessment and practice  
  • Onward referral to other services  
  • Support with returning to work  
  • Low-level mental health support  
  • Carer support  

Who can be seen by the OT Service? 

 The service accepts referrals for individuals aged 18 and over who are willing to engage in occupational therapy. The service cannot see individuals currently under the care of another therapy team or require same-day or next-day services.  

Referral method 

The service is fully embedded within the Cwmtawe Cluster. Referrals can be made directly by any professional within the Cwmtawe GP surgeries. Self-referrals are also accepted. To self-refer, individuals can contact their GP surgery and request an initial call from the Occupational Therapy team.  

What happens next? 

Following a referral, an initial telephone review will be conducted. During this call, the team will discuss the reason for referral, gather background information, and establish goals. This helps determine whether the OT team is the best fit for your needs. If so, a date will be set for the first assessment.  

Swansea Carers Centre is a specialist voluntary organisation providing support to carers and former carers across Swansea.

It offers information, advice and support to make life easier for the carer and person they are looking after, and provides opportunities to meet other carers to share experiences and work together to make improvements.

Arthritis is a common condition that causes pain and inflammation in a joint.

In the UK, millions of people have arthritis or other, similar conditions that affect the joints. Arthritis affects people of all ages, including children.

Osteoarthritis and rheumatoid arthritis are the two most common types of arthritis.

There are support groups available, including ones in the Swansea area.
