BMHS is a not-for-profit organisation focused on education and advocacy to inspire a mentally healthy BAME community by providing mental health and wellbeing support.

Your local area coordinator can help you find support and advice in your community.

They can help you to find out about your community and introduce you to friendly, helpful people within it.

They can help you explore and build on your strengths and can support you to share your skills and gifts with others. They can help you connect with formal services if that is what you feel you need.

Calan DVS is one of the largest domestic abuse charities in Wales and are committed to working in partnership to end domestic violence and abuse.

The charity aims to relieve the distress, suffering and poverty experienced by families exposed to domestic abuse, and to educate the public regarding the causes and effects of domestic abuse and methods of prevention.

As well as providing general support to victims, they have been specifically commissioned to deliver the IRISi programme across Swansea Bay University Health Board. IRISi provides a dedicated pathway for GP practices to refer patients who they feel are victims of or at risk of Domestic Violence and Abuse (DVA).

The programme delivered by Calan includes training and education of GP practice staff, clinical enquiry, care pathways and an opportunity to refer to specialist domestic violence services. It is aimed at women who are experiencing DVA from a current partner, ex-partner or adult family member and provides support, information and signposting for victims and perpetrators.

The purpose of IRIS is to improve health responses to victims and increase practitioner confidence to ask germane questions and report appropriately. The supporting interventions provided by Calan DVS may include referrals to specialist counselling services with no waiting lists, financial advice and accompanying clients to court and solicitors to provide support and advocacy.

Onward referrals have been made to places of safety, refuge, care homes and hostels and supporting the rehoming of clients in local authority housing.

Discharge medicines review

After coming out of hospital, get a review of your medication with a pharmacist to discuss any new medicines and dose changes, possible side effects, interactions with other medicines and the best time to take them.

Sore Throat Test and Treat

This screens people for bacterial or viral throat infections and allows pharmacists to prescribe antibiotics or painkillers, respectively, if appropriate.

Stop smoking scheme

Community pharmacies now offer help to people trying to quit smoking. This scheme gives advice and motivation to people struggling to quite and provides a variety of nicotine replacement therapy to minimise the number of cravings they get.

Medical waste disposal

Patients can bring unwanted or used medication back to the pharmacy to be disposed of safely.

The Dementia Hwb is a one-stop information centre anchored in the heart of the community for all things dementia related.

There are permanent bases in Swansea's Quadrant Shopping Centre between 11am and 3pm, 7 days a week and Aberafan Shopping Centre Monday to Friday 11am to 3pm.

The Dementia Hwb offers information about dementia and signposting to appropriate support.

It has introduced mobile hwbs across Swansea and Neath Port Talbot, with sessions held in a variety of places.

Neath Port Talbot Carers Service was launched in April 2009 to support the unsung heroes who care for loved ones unable to look after themselves.

Not everyone who does this thinks of themselves as a carer, but caring responsibilities make huge demands on their lives. Our aim is to help unpaid carers recognise their caring role and to provide carers aged 18+ in Neath Port Talbot with advice, information and support.

Mental health service user network

Neath Port Talbot Community Voluntary Services are building a Mental Health Service User Network to ensure that people with lived experience are at the heart of how mental health services are shaped in our area.

Care & Repair Western Bay's mission is to ensure that all older people are supported to live in a safe, warm and secure home as independently as possible for as long as possible.

It is a person-centred and problem-led service, which provides a range of advice services for older people in the Swansea and Neath Port Talbot areas.

Cancer Information and Support Services (CISS) is a registered charity which offers emotional support and accurate, up-to-date information for cancer patients and carers across south-west Wales.

The charity aims to provide a flexible cancer support service, offering emotional support and factual information, while avoiding possible barriers to open discussion.

Call 111 press option 2

24/7 mental health support is now available over the phone in Swansea Bay. People living in Swansea and Neath Port Talbot who need urgent support with mental health issues can now call a team of mental health professionals for free, day or night.

Calling 111 and choosing option 2 puts callers in direct contact with a team of 20 mental health professionals based at Neath Port Talbot Hospital.

The service is available for anyone in Swansea and Neath Port Talbot with a mental health concern, including relatives who need advice.

Call 111 Option 2 is running 24 hours a day, seven days a week, offering a triage service and support or signposting as appropriate.

Callers will be supported by fully trained clinicians working as part of a multi-disciplinary team including mental health nurses, psychological wellbeing practitioners and occupational therapists.

Bowel screening looks for bowel cancer before symptoms show. At least nine out of 10 people will survive bowel cancer if it is found and treated early. Bowel screening aims to find cancer at an early stage when treatment is likely to be more effective.

Bowel screening involves completing a home test kit. The screening test looks for hidden blood in your poo.

People aged between 51 and 74 years old registered with a doctor in Wales will be offered bowel screening every two years.

Taking part in bowel screening when invited is something that you can do to look after your health.

Screening is important because it can:

  • Find cancers before symptoms show
  • Treat cancer early, giving you the best chance of survival.

You may feel well even if you have early bowel cancer.
