Promotion of the Community Pharmacy based Common Ailments Scheme (CAS) and Choose Pharmacy

Recruitment of a Community Clinician/Paramedic role

 

Continued development of a multi-disciplinary cluster led Primary Care Mental Health Model, encompassing Mental Health Practitioner role alongside Social Prescribers

Ensuring linkages to Local Primary Mental Health Support Service

Social Prescribing - sign posting patients with low level mental health and social issues to non-clinical services

Commitment to increasing the levels of Childhood Immunisations & Vaccinations

Ensuring required levels of training in relation to IRIS-i are achieved and appropriate referrals are made

Commence delivery of Primary and Community Services joint RPB Carers Plan for improving Carers' early identification; signposting; and support across agreed service areas of each Local Cluster Collaborative

Develop Healthy Lifestyle & Behaviours via provision of Health & Wellbeing Events and targeted social media communications

Continued investment in GP Exercise Referral Scheme (provided by Third Sector)

Continued investment in Cluster Pharmacist role
