About our service

Our aim is to provide evidence-based wound care, working closely with relevant services in order to support your wound healing. With your consent we may communicate with these health professionals regarding your care.

We will work with you in partnership to develop a plan of care that best suits your needs, while also educating you and/or your carer about how to look after your wounds.

With your consent, we monitor the progress of your wound using digital imaging.

The wound clinic service provides a service by scheduled appointment only.

 

Please remember it is your responsibility to attend appointment or to tell us if you cannot attend and obtain prescriptions supplied by or requested by the wound care staff.

  • Your hospital, nurse, community nurse, GP or another health professional can refer you to our service or you can refer yourself. Contact 01792 530783 or email

  • Following Your initial assessment, it is your responsibility to book ongoing appointments.

  • Failure to attend your appointments at wound clinic without contacting us will result in subsequent appointments being cancelled.

  • Late attendance to the clinic may result in not being seen.

  • We will try to offer an appointment as close to your home as possible, however, there may be times where you will need to travel to other clinics for your appointment.

  • Please note that the Community Wound Care Service is not an emergency service. If you suspect that you have a wound infection please contact your GP or 111.

  • Your first appointment will be approximately 40 minutes.
  • Please bring your medical history and list of your current medication.
  • We will carry out a full holistic nursing assessment of your wound care needs.
  • An appropriate management plan will be developed in collaboration with you and initiated for a minimum of two weeks.
  • This will be regularly reviewed with you and/or your carer's involvement.
