Community Resource Teams (CRT)

Speech and language therapists who are part of community teams see patients in their own homes, including nursing and care homes.

Who we can help

  • Adult patients (over 18 years of age) who are bedbound or housebound.
  • Patients with progressive neurological conditions, such as Parkinson's, dementia, and motor neurone disease.
  • Patients with acquired conditions, such as stroke, brain injury and different forms of cancer.

How to access these services

There is one team covering Swansea Council and another for Neath Port Talbot and who you will see depends on where you live.

Swansea CRT

How to access this service

As a community service, we are linked to social services and thus our referrals are received via CAP (Common Access Point). 

Information on Common Access Point (CAP) can be found on the Swansea Council website here.

Swansea CRT only accepts referrals from registered professionals. If you wish to make a referral to our service, please talk to your GP or health professional, and they will be able to help.

Neath Port Talbot CRT

Postal address: Cimla Hospital, Neath, SA11 3SU.

How to access this service

  • Your GP and other medical, health and social care professionals can make a referral for you.
  • You can refer yourself as the patient.
  • Someone you know, for example a family member or carer, can refer with your consent or in your best interests where they have medical power of attorney (health and welfare attorney).

When making a referral, please include as much information as possible about the reason for the referral. This means your request can be triaged and prioritised by urgency of your need.

If we cannot help with your referral, we will let you know. Where possible, we will recommend which other service may be best placed to help you.
