Face masks and social distancing: Due to the rising prevalence of COVID-19 in our communities, we strongly encourage healthcare staff and visitors to wear a face covering in all of our settings, particularly in clinical areas and those with high footfall. Please exercise a common-sense approach and personal responsibility to help us reduce the impact of COVID-19 on our patients, workforce and services. In addition to wearing a face covering, it is important to continue to maintain social distancing where possible. Thank you for your continued support and co-operation at this time. We continue to regularly review our advice based on prevalence in our communities and our hospitals.

Physio Access Clinic (PAC)

We will be starting a trial of a new Physiotherapy Access clinic. This is a face-face assessment clinic that will initially be held in the Baglan Resource Centre (SA12 7BJ). PLEASE PRINT AND COMPLETE THE SUBJECTIVE ASSESSMENT FORM. (Please follow this link to download the Subjective assessment form.). IF YOU DO NOT HAVE A PRINTER, PLEASE ARRIVE 10 MINUTES EARLY TO FILL IN THE PAPERWORK. Appointments will be 20 minutes and must be booked prior to arrival. For the initial trial, the clinic will be open every morning to begin. Booking information can be found at the bottom of this page. Appointments will be available for three days in advance. If the clinic is full, we offer alternative phone call assessments daily on the information at the bottom of this page.

This clinic is suitable for those above the age of 16, not already under the physio service and would like assessment for a musculoskeletal problem (Bone, joint, ligament, nerve etc). 

Prior to attending your appointment, we ask you to read the information sheet.  Please follow this link to download the covid information form. By arriving to your appointment it will be presumed that you have consented to the above information. 

Additionally; We would please request that you complete and bring the attached sheet with you. Please follow this link to download the Subjective assessment form. This will enable us to maintain a time effective clinic and prevent over running appointments. This information is crucial for our assessment. 

We appreciate the Baglan resource center may be difficult for some patients to attend. If successful, we will look to expand this service to a 5 day service and potentially across sites.
