Support Services for Vulnerable People in South West Wales during COVID 19 outbreak

Emergency Contact Centre – 01792 636 363

This is a new team set up to provide support to patients who have received a letter advising them they need to be shielding for the next 12 weeks and don’t have any support from family, friends or neighbours.

  • The team will provide:
    • Sign posting to emotional support services during this difficult time.
    • Support with GP medication deliveries
    • Support with food deliveries and packages

Swansea Council Voluntary Service (SCVS)

SCVS are co-ordinating a large-scale voluntary effort to support and sign post patients to the different services that are available locally across Swansea. They will not be supporting patients who have received a shielding letter, but will be supporting other vulnerable people in local communities. There are local co-ordinators for each area, have a look on the link above for that information.

  • They have said we can sign post people directly to SCVS for emotional support during this time, they have befriending services running across Swansea who they can sign post people to of various ages.

Neath Port Talbot Safe & Well Service – 01639 686868 –

This is a free service to support vulnerable residents who have been told to self-isolate and don’t have any support from family, friends or neighbours. The service can arrange for a trusted volunteer to help with daily living tasks such as food shopping, collecting medical supplies, posting mail and any other reasonable tasks. All the volunteers will have proof of identity and will not ask for any money or bank details.

Carmarthen Council Coronavirus Support Service Number - 01267 234567

Carmarthen are providing additional support to help and advise Carmarthenshire businesses and residents throughout the on-going Coronavirus crisis. You can connect to this new service by calling us on 01267 234567. This is available between 8.30am and 6pm seven days a week.

Pembrokeshire Council Community Hub Team - 01437 776301

Pembrokshire Council have a webpage set up to help guide people to the right form of support as quickly as possible. They also have a community Hub Team set up to responds and deal with the crisis at this time, they can be contacted on 01437 776301 or via email on communitycovid19@pembrokeshire.gov.uk

A Ceredigion County Council Gold Command group continues to meet and work with partner agencies including Public Health Wales to prepare and respond to this emerging situation. They have a ‘Keeping in Touch Service’ set up which can be accessed by phone 01545 570881 or email clic@ceredigion.gov.uk

The Kidney Wales Foundation - 02920343940 

The Kidney Wales charity have a new Kidney Wales Community Facebook Page which patients can request to join, get support and up to date information.

  • Kidney Wales also have the CanDo Support Page, which can be found on Facebook where young patients can access support from patient advocates and meet other patients online.
  • Kidney Wales are working with hospital staff to provide Zoom (Video calling support sessions) session over the coming weeks.
