The post of Cluster Paramedic was created in January 2018 and at the time was the first of its kind in clusters within Wales.
The primary role of the paramedic is to undertake home visits to patients on behalf of all eight GP Practices in the cluster.
The paramedic works on his own in providing clinical assessments, diagnosis, treatment and referral and can signpost patients to services within the cluster area.
The role involves close communication with all colleagues across the cluster and has a crucial part to play in continuity of care and helping to reduce the pressure on GP practices.
Swansea Bay’s primary care audiology services delivers quicker specialist access for patients in the community.
Patients with hearing problems, tinnitus or problematic wax can now phone their surgery’s telephone triage system and book directly to see one of the primary care audiology teams at designated clinics.
It replaces the previous system which included a surgery appointment with a GP or practice nurse, who would then refer the patient to the audiology team.
The service within City Health Cluster is hosted at Beacon Centre for Health (SA1, Swansea).
The service is in line with the Welsh Government’s vision for transforming primary care across Wales, and the emphasis on providing a range of quality health and social care services in the community.
It also offers advice and directs patients to information that will help them make decisions on their hearing care and manage the effects of their hearing loss and tinnitus.
The All-Wales Diabetes Prevention Programme, which launched in 2022, targets people who are found to be prediabetic, or at high risk of becoming diabetic, and helps them make the necessary lifestyle changes to avoid developing the disease.
The programme offers patients a 30-minute consultation with a specially trained dietetic support worker.
This focuses on topics such as physical activity, healthy eating and promotes other lifestyle changes such as smoking cessation and alcohol reduction.
The programme is now available in all eight of the Swansea Bay clusters – with a dietetic support worker based in each.

Emerging evidence shows that social prescribing leads to a range of positive health and wellbeing outcomes, such as improved quality of life and emotional wellbeing.
In addition, research shows that social prescribing can reduce the amount of GP appointments and hospital admissions.

The social prescribers provide provide fast access to information and support for people who can be supported better in their own community and they complement other approaches in a local area such as active signposting, local knowledge and resource directories. As such patients in City Health Cluster are better informed of what is available to them in their own community.
