Social prescribing - Sign post patients with low level mental health and social issues to non-clinical services.

Third Sector provision - Commissioning of a varied range of emotional and wellbeing services in the Cluster.

Wellbeing offer including psychological therapies  for adults, children and young people commissioned.

Engage with Child and Adolescent Mental Health Services (CAMHS) around referrals, support pilot for link services.

Develop comprehensive wellbeing offer for low levels of Mental Health.

Suicide prevention – evaluation of project to improve identification and referrals.

Delivery of annual health checks for patients with learning difficulties.

 

Promoting nutrition in schools – Healthy Eating project through Swansea Foundation addressing childhood obesity.

Increasing childhood Immunisations & vaccinations.

 

Safeguarding – peer support enhancement & improved protocols.

Explore expanding IRIS-i  across all Health Professions in the Cluster.

Weight Management including:
- GMS & Community Pharmacy deliver dietary education and support.

Co-production.

Develop strategies which contribute to the decarbonisation of the environment to help with the current climate emergency.

Commence delivery of Primary and Community Services joint RPB Carers Plan for improving Carers' early identification; signposting; and support across agreed service areas of each LCC.

Improve safety of antipsychotic prescribing working with Medicines Management and Mental Health.
